The author has taken on the gargantuan undertaking of assembling a totally revamped and updated third edition text. Remarkably, this book goes beyond and exceeds editions that have come before. Color photographs and illustrations are well choreographed, insightful, and clear. Likewise, the writing of each chapter is concise, substantially informative, and easily assimilated. The book showcases tried and true procedures that afford a predictable and reproducible outcome, especially since potential complications are part and parcel of any intervention.
